Leading the game at the half, Italy faded early in the third before coming back on a pair of Andrea Amato threes late in the frame.
But when Jose Nogues contributed his first points of the night with a buzzer-beating tip in, Spain were back in front by five with 10 minutes to go.
Following it up with another buzzer-beating shot, this time a three-pointer to beat the shot clock with under two minutes gone in the deciding frame, Nogues had Spain up by 12 and Italy's valiant resistance was broken.
Italy took over five minutes to score their first points in the final frame, before eventually managing 13 for the quarter.
Ilimane Diop Gaye topped Spanish scoring with 14 points alongside eight rebounds.
Guillermo Hernangomez was next best with 12 while Sebastian Saiz had eight points and nine rebounds.
Italy's Simone Fontecchio lead all scoring with 15 points.
